In order to improve the consistency and quality of the field data, a new field data collection form has been released. The new form is available for download below. Field data should be submitted via the online data entry form, which has been updated to reflect the new field data form.
If you have any questions regarding how to use the new field data form, or how to submit your data online, please contact the monitoring coordinator (304-329-3621 or madison@cheat.org). Additionally, there is space provided at the end of the online data form for your questions/comments. Please utilize this space, as it is our intention to make the online data entry as user-friendly as possible.
**In order to submit your data online, a 5-digit user ID is required. If you do not have, or are unsure of your user ID, please contact the monitoring coordinator.
